Abstract Aim: To evaluate canal transportation and centering ability of Protaper and WaveOne systems in curved root canals by cone-beam computed tomography. Methods: Twenty human molars were randomly divided into two groups according to the system used: Group GP (ProTaper Universal(r)) and group GW (WaveOne(tm)). CT scans were performed before and after the chemo-mechanical preparation on three points: 2 mm, 3 mm and 4 mm from the tooth apex. The cuttings in dentin were measured and results were statistically analyzed. Results: In GP, there was higher percentage in transport at 2 mm and 4 mm toward the mesial wall of the tooth root compared to GW (p<0.05). There was no difference between the systems regarding their centering ability. Conclusions: None of the systems was able to touch all the dentin walls and stay centered during the chemical-mechanical preparation.

OBJETIVO: Avaliar in vitro a capacidade seladora de três materiais utilizados como barreira cervical durante o clareamento interno. MÉTODOS: Foram selecionados trinta e três caninos humanos, que após o preparo biomecânico dos canais radiculares, foram obturados pela técnica da condensação lateral ativa. Após a reação de presa do material obturador os dentes foram divididos aleatoriamente em três grupos de dez dentes cada, de acordo com os seguintes materiais para confecção da barreira cervical: X Temp LC; Vitro Fil; Coltosol. O grupo controle constituiu de três dentes sem a confecção do tampão cervical. Utilizou-se como substância clareadora o Perborato de Sódio/ água destilada associado à Rodamina B 2%. Após a colocação do agente clareador na câmara pulpar, as aberturas coronárias foram seladas com cimento provisório IRM e as amostras foram armazenadas em estufa a 37° por 7 dias. Após esse período, as amostras foram clivadas no sentido longitudinal, e a avaliação da infiltração foi realizada pelo método visual. Os dados foram submetidos ao teste estatístico de Kruskal-Wallis, com nível de significância de 5%. RESULTADOS: Não houve diferença estatisticamente significante entre os grupos de estudo, embora o Coltosol tenha obtido maior número de espécimes com menor grau de infiltração. CONCLUSÃO: Todos os materiais avaliados na confecção do tampão cervical permitiram algum nível de infiltração.

OBJECTIVE: To evaluate in vitro the sealing ability of three materials used as cervical barrier for internal dental bleaching. METHODS: Thirty-three canines were selected, and after biomechanical root canal preparation, the canals were filled by lateral condensation technique. After the filling material setting reaction, the teeth were randomly divided into three groups of ten teeth each, according to the following materials for constructing the cervical barrier: X Temp LC; Vitro Fil; Coltosol. The control group consisted of three teeth, without placement of the cervical cap. Sodium perborate /distilled water associated with 2% Rhodamin B were used as bleaching substance. After placing the bleaching agent in the pulp chamber, access cavities were sealed with temporary cement IRM and samples were stored at 37 ° for 7 days. After this period, the samples were cleaved longitudinally, and leakage was evaluated by the visual method. The data were submitted to Pearson test with a 5% level of significance. RESULTS: No statistically significant difference was found between the study groups, however, the largest number of specimens with a lower degree of leakage were found in the Coltosol group. CONCLUSION: All the materials evaluated for making the cervical cap allowed some level of leakage.

O objetivo desse estudo foi verificar por meio da espectroscopia Raman, a existência de correlação entre os níveis de apatita fosfatada (∼960 cm−1), apatita fluoretada (∼575 cm−1) e matriz orgânica (∼1450 cm−1) e as leituras do Diagnodent® em diferentes estágios de cárie dental em dentes humanos extraídos. O valor médio do pico de fluorescência na área da cárie foi anotado e os dentes divididos em cárie de esmalte, dentina e dente hígido. Após as leituras de fluorescência, foi realizada a espectroscopia Raman nos mesmos sítios. Os resultados mostraram diferença significante (ANOVA p<0,05) entre as leituras de fluorescência para esmalte (16,4 ± 2,3) e dentina (57,6 ± 23,7) nos dentes cariados. Os picos Raman para esmalte e dentina evidenciaram que os picos ∼575 e ∼960 cm−1 foram mais intensos em cárie de esmalte. Houve correlação negativa e significante (p<0,05) entre os picos ∼575 e ∼960 cm−1 e cárie de dentina. Pode-se concluir que quanto maior a fluorescência detectada pelo Diagnodent menor o pico da apatita fosfatada e fluoretada. O diagnóstico precoce da cárie está diretamente relacionado com a identificação de mudanças nos componentes inorgânicos do dente, assim a espectroscopia Raman foi mais sensível para variações desses componentes quando comparada ao Diagnodent.

The aim of this work was to verify the existence of correlation between Raman spectroscopy readings of phosphate apatite (∼960 cm−1), fluoridated apatite (∼575 cm−1) and organic matrix (∼1450 cm−1) levels and Diagnodent® readings at different stages of dental caries in extracted human teeth. The mean peak value of fluorescence in the carious area was recorded and teeth were divided in enamel caries, dentin caries and sound dental structure. After fluorescence readings, Raman spectroscopy was carried out on the same sites. The results showed significant difference (ANOVA, p<0.05) between the fluorescence readings for enamel (16.4 ± 2.3) and dentin (57.6 ± 23.7) on carious teeth. Raman peaks of enamel and dentin revealed that ∼575 and ∼960 cm−1 peaks were more intense in enamel caries. There was significant negative correlation (p<0.05) between the ∼575 and ∼960 cm−1 peaks and dentin caries. It may be concluded that the higher the fluorescence detected by Diagnodent the lower the peaks of phosphate apatite and fluoridated apatite. As the early diagnosis of caries is directly related to the identification of changes in the inorganic tooth components, Raman spectroscopy was more sensitive to variations of these components than Diagnodent.

A mucosite oral é um efeito colateral prejudicial da radioterapia na região de cabeça e pescoço. Existem estudos que evidenciam o efeito benéfico do uso da luz laser no tratamento da mucosite oral. O objetivo deste caso clínico foi o de avaliar a eficácia da fototerapia laser no tratamento da mucosite oral em um paciente sendo submetido a radioterapia, após a remoção cirúrgica de um carcinoma escamocelular, com invasão óssea da maxila. As lesões do palato e das comissuras labiais foram tratadas com λ660 nm, 40 mW, ∅=4 mm², em contato, 5 x 2.4 J/cm² por ponto, 14.4 J/cm² por sessão. Na lesão existente na mucosa nasal a fototerapia laser (∅=4 mm², λ780 nm, 70 mW, 3 x 2.1 J/cm² por ponto, 6.3 J/cm² por sessão, em contato) foi utilizada na área externa do nariz. Uma dose única (2.4 J/cm²) com o laser λ660 nm e os parâmetros descritos anteriormente foi aplicado na entrada de cada narina. A fototerapia laser foi utilizada 3 vezes por semana, durante 4 semanas. Os resultados do tratamento são indicativos de que o uso da fototerapia laser em mucosite oral foi efetiva e permitiu ao paciente continuar o tratamento radioterápico sem interrupções. Entretanto, estudos clínicos controlados são necessários para se estabelecer os protocolos, para tratamento e prevenção da mucosite oral, utilizando fototerapia laser.

Oral mucositis is a harmful side effect of radiotherapy (RT) on the head and neck region. There are encouraging reports on the beneficial aspects of the use of laser light on the treatment of oral mucositis. This paper reports the efficacy of laser phototherapy (LPT) on the treatment of oral mucositis in a patient undergoing RT after surgical removal of a squamous cell carcinoma with osseous invasion of the maxilla. Palatal and commissural lesions were treated with λ660 nm, 40 mW, ∅=4 mm², in contact mode, 5 x 2.4 J/cm² per point, 14.4 J/cm² per session. For treating the lesion on the patient's nasal mucosa, LPT (∅=4 mm², λ780 nm, 70 mW, 3 x 2.1 J/cm² per point, 6.3 J/cm² per session, contact mode) was used on the external area of the nose. A single dose (2.4 J/cm²) with the λ660 nm laser, as described before, was applied on the entrance of each nostril. LPT was used 3 times/week during 4 weeks. Treatment results indicate that the use of LPT on oral mucositis was effective and allowed the patient to carry on the RT without interruption. However, long-term and controlled clinical trials are necessary to establish both preventive and curative protocols using LPT.

OBJECTIVE: The aim of this study was to evaluate the biomechanical preparation of flattened root canals using the following systems: Endo-Eze AET stainless steel oscillatory instruments (Ultradent) and RaCe rotary NiTi instruments (FKG Dentaire). MATERIAL AND METHODS: Twenty extracted human mandibular incisors were randomly assigned to two groups: Group 1 - Instrumentation with oscillatory Endo-Eze AET files (oscillatory technique); Group 2 - Instrumentation with rotary NiTi RaCe files (rotary technique). The teeth were decoronated, had their apices and coronal openings sealed with sticky wax and were embedded in crystal-clear orthophtalic polyester resin. The roots were sectioned transversally with diamond discs at 10 mm (middle third) and 5 mm (apical third) from the apex and the segments were reassembled for instrumentation. The sections were photographed before and after root canal instrumentation and evaluated with respect to whether the original root canal shape was modified by instrumentation. To evaluate the differences in the root canal shape before and after biomechanical preparation, scores were given regarding the instruments touch on the intracanal walls. RESULTS: In middle third of the root canals instrumented with the rotary system, there was a change in the original canal anatomy (p<0.05), with formation of a protuberance in the mesiodistal direction. This protuberance did not occur when the oscillatory instrumentation was used. The oscillatory system had better results in the middle and apical thirds as evaluated by Dunn's multiple-comparison test (p>0.05). CONCLUSION: Under the tested conditions, Endo-Eze oscillatory system yielded the instrumentation of all flattened root canal walls, maintaining the canal original shape throughout the biomechanical preparation, and was more effective than RaCe rotary system.
